Before Your First Visit

For Counseling:

1. Contact your insurance carrier to ensure that your provider clinician is covered by your plan.

2. Get an authorization number.

3. Get an authorized number of sessions.

4. Get your co-payment amount.

5. Bring this information with your co-payment to your visit(s).
